i don't think they can *tell*, perse, since not every girl is born with a hymen, but they will ask. so if you're worried about your mother finding out (they're required by law to keep your confidentiality) then find a way to not have your mother in the room when they ask.
Anyway, if you've picked up an STD on any of those 5 times, the doctor might have a good guess that you've been active. Genital warts or herpes sores are a good giveaway.
But usually they do *ask* whether you're sexually active and how long you've been with your current partner, because they need to know whether to screen you for chlamydia/gonorrhea/syphilis/AIDS/etc.
